• Parents please use this form to provide feedback on your 2009-2010 season
• Place completed form (labelled Rep-AE or Local League) in the LMHA mailbox in Pad 1 at the Lindsay Recreation Complex

Coach Personal Evaluation Form for Coaches and Assistant Coaches

• Coaches please use this form to provide feedback on your 2009-2010 season
• Use by Assistant Coaches is optional
• Place completed form (labelled Rep-AE or Local League) in the LMHA mailbox in Pad 1 at the Lindsay Recreation Complex
